Entries close by May 30, 2009. Voting period ends July 1, 2009. Winner Announced on or before August 7, 2009.
Prizes:
- 1st place - $200 and free marketing of your winning photo to dental companies and modeling agencies for 1 year. Cover person and spokesmodel for SmileStruck products (paid position) for from Jan 1, 2010 until December 30, 2010.
- 2nd place - $50.
- 3rd place - $25.
How Do I Apply?
Follow this free 3 step process:

2. Update your profile and Upload 1 picture of your best smile .
3. Have your friends, family and anyone else vote for you online by accessing your profile.
Judging:
Judging will be based on the number of online votes received from the community of people accessing our web site. Voters can be from anywhere in the world.
Rules & Restrictions:
NO PURCHASE NECESSARY. To enter, complete the entry form after creating a free online account. To be eligible, entries must be completed and received on or by the closing date of the draw. Incomplete entries will be disqualified. All entries shall become the property of Smile Struck LLC. Smile Struck LLC is not responsible for lost, misdirected or delayed entries. Entries received by telephone, fax, courier or personal delivery will not be accepted. Contestants may only place their own picture on our web site and must agree and adhere to our terms of conditions and privacy policy. Contestants may update their picture at any time through marketing period. Only contestants who input their address, city ,state, zipcode, country, telephone # and email address, uploaded photo in their online profile will be considered to have a complete application. Contestants may not place copyrighted material as their photo.
The contest is open to the general public as well as of anyone of age of majority. Identification must be produced on request. Employees of Smile Struck LLC, its contest sponsor, advertising and promotional agencies and their respective affiliates and associates and such employees' immediate family members and persons with whom such employees are domiciled are excluded from this contest.
By claiming the prize, the winner authorizes the use, without additional compensation of his or her name and/or likeness and/or voice/photograph and municipality of residence for promotion and/or advertising purposes in any manner and in any medium (including without limitation, radio broadcasts, newspapers and other publications and in television or film releases, slides, videotape, distribution over the internet and picture date storage) which Smile Struck LLC may deem appropriate.
In accepting the prize, the winner, and any guest(s), acknowledges that Smile Struck LLC may not be held liable for any loss, damages or injury associated with accepting or using this prize(s).
Detailed contest rules applicable to this contest, including contest entry dates, how to enter, and prize value, number and any restrictions applicable to these prizes are available upon request from Smile Struck LLC.
Winners and any guests at the age of majority must sign a release form as prepared and accepted by Smile Struck LLC acknowledging their acceptance and understanding of the complete contest rules.
Smile Struck LLC retains the rights, in its absolute and sole discretion, to make substitutions of equivalent kind or approximate value in the event of the unavailability of any prize or component of the prize for any reason whatsoever.
This contest is subject to all federal, provincial and municipal laws.
Smile Struck LLC reserves the right to withdraw or terminate this contest at any time without prior notice.
If prize involves travel, winner and travel companion must agree to the airline's Terms and Conditions of travel. Once confirmed, trip dates and travel accommodations cannot be changed by winner; however, travel is subject to availability, blackout dates, and/or change due to circumstances beyond Sponsors' and Administrators' control. Winner and travel companion must travel on the same dates, itinerary, and fare class. No extensions will be permitted, and any unused travel will be forfeited. Prize cannot be used in conjunction with any other promotion or offer and may not be separated. All tickets issued in conjunction with the Grand Prize are not eligible for frequent flyer miles, and no code-share flights may be used.
One entry per person.
Contest open to resident of any country. Payment will be made in US Dollar only.
The prize is not redeemable in cash and must be accepted as awarded.
Voting: Only one vote per IP address will be recorded in the voting system. Multiple votes from the same IP Address will be discarded. Decisions of the contest judges are final - no substitutions will be available. Judging will be done by online community that accesses web site. Profile with the most votes will win contest. Smile Struck LLC reserves the right to terminate any entry that is suspected or thought to use automated bots or other automated programs that fraudently increase number of votes. Only one vote per IP Address will be permitted. Only one vote per person is allowed.
